Nomination Forms/Instructions for 2008

ATTENTION NOMINATORS
Now is the time to submit nominations to recognize fellow employees
for implementing adaptable award winning achievements
that have saved time, money and resources

Please contact your Agency Coordinator for information on submitting a Prudential - Davis Productivity Awards nomination. Nominations will only be accepted from an authorized DPA Agency Awards Coordinator. Your Agency Coordinator has a deadline for submitting the awards. (All nomination forms and instructions are available in Microsoft Word.)

The deadline for submitting nominations to your Agency Awards Coordinator is Friday, September 28, 2007. (Note: Individual agency deadlines may vary; therefore, please verify your respective agency's submission deadline date with your Agency Coordinator.)

Cash awards of $250 to $2,500, commemorative plaques, and certificates of commendation will be presented to state employees in June 2008. Please nominate individuals, teams, work units and agencies that exceeded their job descriptions and performance expectations in ways that improved service delivery and added value for Florida taxpayers and businesses between October 1, 2006 and September 30, 2007 or other comparable period.

Please submit nominations for Individuals, Teams, Work Units and Partnerships using the NEW Online Nomination System.

Through the Online Nomination System, you will be able to create nomination forms, save your work for future editing and expedite the review and approval process by your manager, agency awards coordinator and agency head.

Should you have any questions regarding how to use this system, please contact your agency awards coordinator.

During this first year of using the Online Nomination System, only Individual, Team, Work Unit and Partnership Nominations will be accepted through the system. However, our goal is to transition into having all nominations submitted through the system for the 2009 competition.

Exemplary Achievement of a State Agency Nomination -- This form is to be used to nominate an Agency, Division, District, Institution or comparable organization.

Adapting and Implementing Nomination -- This form is to be used to nominate an individual, team, or work unit for adapting or implementing a previous Prudential - Davis Productivity Award.

Sustained Exemplary Performance Nomination -- This form is to be used to nominate previous DPA winners who have demonstrated exemplary performance for five years or longer. There are separate forms for nominating an individual versus nominating a team, work unit, district, institution or comparable organizational unit.

Recognition Of Long Time Nominators - This form is to be used to nominate individuals who have submitted nominations to the Davis Productivity Awards program over a period of ten or more years.

If your nominee moves to another job within your agency, or another agency, or leaves state employment between the time your nomination is submitted and the awards presentations, or if there is any other significant change in the status of the nominee or the nomination, disbursement of the award will be at the discretion of the agency.
